The CliniSyncPLUS team has been working closely with the Ohio Department of Health to explore chronic conditions in Ohio, specifically hypertension
and diabetes. Working with practices throughout the state, they not only researched the extent of these conditions but suggest ways in
which practitioners can manage patient populations who have these illnesses. This work is supported by Cooperative Agreements #5NU58DP005508
and #6NU58DP004826, funded by the Centers for Disease Control and Prevention. Its contents are solely the responsibility of the authors and
do not necessarily represent the official views of the Centers for Disease Control and Prevention or the Department of Health and Human Services.
